Age UK Lancashire’s Living Well Support Service is designed to help older people return home safely after a hospital stay, prevent unnecessary hospital admissions, and provide urgent support following a significant life event. Our goal is to promote independence, improve wellbeing, and ensure you have the right support to live safely and comfortably at home.
We offer two levels of personalised support:
Tier 1 – A short-term service (up to 3 hours) that includes a home assessment to identify any ongoing support needs, with the option of transport home from the hospital.
Tier 2 – A follow-up support package (up to 15 hours) providing essential information, advice, and practical assistance. This includes home safety checks, mobility aids, falls prevention, medication support, care needs assessments, finance and benefits advice, fire safety guidance, and access to local support networks.
