Age UK Lancashire’s Living Well Support Service is designed to help older people return home safely after a hospital stay, prevent unnecessary hospital admissions, and provide urgent support following a significant life event. Our goal is to promote independence, improve wellbeing, and ensure you have the right support to live safely and comfortably at home. We offer two levels of personalised support: Tier 1 – A short-term service (up to 3 hours) that includes a home assessment to identify any ongoing support needs, with the option of transport home from the hospital. Tier 2 – A follow-up support package (up to 15 hours) providing essential information, advice, and practical assistance. This includes home safety checks, mobility aids, falls prevention, medication support, care needs assessments, finance and benefits advice, fire safety guidance, and access to local support networks. Our dedicated team ensures you receive tailored support to regain confidence and independence at home. Description

At Progress Lifeline, we provide a range of telecare sensors that can be installed in homes to protect those who live alone or are vulnerable in any way. Our products are designed to sense danger in the home or vulnerability to falls and identify the need for assistance. These sensors are all linked to our 24/7 Alarm Receiving Centre where our extremely friendly and helpful team will check you are safe and call the appropriate help if needed. We also provide an Emergency Response Service. The emergency responders are available 24 hours a day, 365 days a year to visit you if you are in need, or if we cannot confirm you are safe following an activation. Our responders will get to you within 60 minutes, although our average is normally 35 minutes. Progress Lifeline can also monitor people outside of the home with our GPS device which can be worn as a pendant or a key-fob. The GPS device, named The Footprint can provide extra reassurance inside and outside the home or if a person does not have a telephone line they can use this for connection to our Alarm Receiving Centre. Offices are open Monday to Friday 9-5pm Alarm Receiving Centre is open 24 hours a day, every day of the year.

We are a local not-for profit organization that has been providing practical support and independent advice since 1993. We work with older, vulnerable and disabled people and anyone with a long term health condition that affects their mobility of independence in the home. We work across Chorley, Preston, South Ribble and West Lancashire.   We offer impartial advice and practical help including: ·         Handyperson & Minor Works Service. ·         Healthy Home Checks to improve home safety and security. ·         Advice and assistance with larger adaptations and home repairs. ·         Practical support to people returning home from hospital. ·         Finding trusted contractors and getting quotes for work. ·         Assistance with filling in forms and applying for financial help for adaptations and repairs. ·         Advice and support to help you make the right housing choices for your needs.
