  We provide support to the Deaf Community in Lancashire. Sue Gardam is the Deaf Support Link Worker for n-compass and covers the whole of Lancashire. This role has 3 main areas of responsibility: 1. To visit culturally deaf adults and work with them to overcome communication barriers that they face when trying to access services, which are having a negative effect on their wellbeing. 2. To work with services and professionals to share with them the information that they need regarding the communication needs of the deaf community. 3. To create and establish effective pathways into services that are reliable and sustainable, and share this information with professionals and the deaf community. Sue is not an interpreter and this is not an interpreter service. People can be referred into the service if they are BSL users and are facing barriers to access services such as health, social services, housing etc., or barriers which they are unable to overcome and are having a negative affect on their wellbeing.

Description

At Progress Lifeline, we provide a range of telecare sensors that can be installed in homes to protect those who live alone or are vulnerable in any way. Our products are designed to sense danger in the home or vulnerability to falls and identify the need for assistance. These sensors are all linked to our 24/7 Alarm Receiving Centre where our extremely friendly and helpful team will check you are safe and call the appropriate help if needed. We also provide an Emergency Response Service. The emergency responders are available 24 hours a day, 365 days a year to visit you if you are in need, or if we cannot confirm you are safe following an activation. Our responders will get to you within 60 minutes, although our average is normally 35 minutes. Progress Lifeline can also monitor people outside of the home with our GPS device which can be worn as a pendant or a key-fob. The GPS device, named The Footprint can provide extra reassurance inside and outside the home or if a person does not have a telephone line they can use this for connection to our Alarm Receiving Centre. Offices are open Monday to Friday 9-5pm Alarm Receiving Centre is open 24 hours a day, every day of the year.

Provides information and support to adults affected by cancer either directly with a current or previous diagnosis of cancer. This can be from diagnosis, through treatment and for those people who suffer the long term consequences of treatment. We undertake holistic needs assessments to identify the physical, psychological, social and spiritual needs, develop a care plan with the person and provide ongoing support. We are social prescribers working collaboratively with many agencies to meet the needs of our service users. We have two support groups: General support group on the 1st Monday of each month Carers support group 1st Tuesday of each month. We can signpost onto cancer specific support groups too. Outreach points are Parbold Surgery 1st Wednesday of the month 9.30-11.30 North Meols Medical centre banks, 2nd Wednesday of the month Tarleton Health Centre Last Friday of the month 9.30-11.30 Provide literature and internet access on all aspects of living with cancer including travel and work issues. The opportunity to talk to someone in a friendly non clinical environment. Help with accessing financial advice and grants. Information about practical help which is available. Free confidential drop in service for anyone affected by cancer. This includes people who reguire information about cancer, have a cnacer diagnosis, are living with the consequences of a cancer diagnosis, are living with and beyond cancer, are looking to reduce their risk of cancer, are a carer relative friend of someone with cancer, are health & social care professionals. A care plan is developed following a physical, psychological, social and spiritual assessment. We aim to provide a safe, welcoming environment for those who are experiencing social isolation because of their cancer diagnosis.

We support often hard-pressed Carers to reduce the impact of their caring role through providing information along with specialist 1-2-1 and group support including: Magazines twice a year detailing local groups, activities and courses. Regular Coffee and Chat support groups in locations across North and Central Lancashire. Carers Assessments and reviews. We can support you, in accordance with Care Act 2014, to undertake a Carers Assessment to identify the impact your caring role may be having on your physical and emotional health and wellbeing, whether you are willing and able to continue in your caring role and, if so, what support is needed for you to do this. We will help you to create a plan of how to put this support in place. We will also assess whether you are eligible for a Carers Personal Budget to spend against your plan. Support with planning for contingency or an emergency. We can support you to create a contingency care and support plan, including an option for up to 72 hours free replacement care for emergencies where you are taken into hospital unexpectedly or are involved in an incident involving emergency services and no support is available from family or friends. Support to access community and Health and Wellbeing services. Access to a 24-hour CHAT Line manned by volunteers who can offer understanding with regards to the common challenges faced by Carers. The CHAT Line is available 24 hours a day, 7 days a week, 365 days a year. Access to our Volunteer Befriending and Sitting-In services where volunteers are matched with Carers and the people they care for to provide company and a break from caring. Support for former Carers. Volunteering opportunities for Carers, including volunteering for the CHAT Line, Befriending and Sitting-In services. This team provides the following three services Falls Prevention (general referrals), Community Physio (GP Referrals only), and Rapid Assessment (which can only be referrals from health and social care professionals). Falls Prevention Service referrals should be made by visiting http://www.lancashirecare.nhs.uk/Community-Rehab completing referral form and attaching this to referral. The referral will be declined if the referral form is not completed in full. The service provides a specialist service for adults who have fallen or who are at risk of falling. Patients are visited in their own home and a multifactorial assessment to check for environmental and other contributing falls risks is completed. People may be assessed by an Occupational Therapist and/or a Physiotherapist. We will discuss options for reducing risks of falls and agree a care plan for any on-going treatment or equipment. We also operate an open referral system through the Main Access Point for NHS community referrals 01772 777999. After receiving your referral we may ring the patient to triage the situation and either allocate directly to a therapist or place referral on our waiting list. We endeavour to see people according to need and within 8 weeks where possible.
