Description
Provides information and support to adults affected by cancer either directly with a current or previous diagnosis of cancer. This can be from diagnosis, through treatment and for those people who suffer the long term consequences of treatment. We undertake holistic needs assessments to identify the physical, psychological, social and spiritual needs, develop a care plan with the person and provide ongoing support. We are social prescribers working collaboratively with many agencies to meet the needs of our service users. We have two support groups: General support group on the 1st Monday of each month Carers support group 1st Tuesday of each month. We can signpost onto cancer specific support groups too. Outreach points are Parbold Surgery 1st Wednesday of the month 9.30-11.30 North Meols Medical centre banks, 2nd Wednesday of the month Tarleton Health Centre Last Friday of the month 9.30-11.30 Provide literature and internet access on all aspects of living with cancer including travel and work issues. The opportunity to talk to someone in a friendly non clinical environment. Help with accessing financial advice and grants. Information about practical help which is available. Free confidential drop in service for anyone affected by cancer. This includes people who reguire information about cancer, have a cnacer diagnosis, are living with the consequences of a cancer diagnosis, are living with and beyond cancer, are looking to reduce their risk of cancer, are a carer relative friend of someone with cancer, are health & social care professionals. A care plan is developed following a physical, psychological, social and spiritual assessment. We aim to provide a safe, welcoming environment for those who are experiencing social isolation because of their cancer diagnosis.
