Description

At Progress Lifeline, we provide a range of telecare sensors that can be installed in homes to protect those who live alone or are vulnerable in any way. Our products are designed to sense danger in the home or vulnerability to falls and identify the need for assistance. These sensors are all linked to our 24/7 Alarm Receiving Centre where our extremely friendly and helpful team will check you are safe and call the appropriate help if needed. We also provide an Emergency Response Service. The emergency responders are available 24 hours a day, 365 days a year to visit you if you are in need, or if we cannot confirm you are safe following an activation. Our responders will get to you within 60 minutes, although our average is normally 35 minutes. Progress Lifeline can also monitor people outside of the home with our GPS device which can be worn as a pendant or a key-fob. The GPS device, named The Footprint can provide extra reassurance inside and outside the home or if a person does not have a telephone line they can use this for connection to our Alarm Receiving Centre. Offices are open Monday to Friday 9-5pm Alarm Receiving Centre is open 24 hours a day, every day of the year.

Community Rehabilitation Team

Description

This team provides the following three services Falls Prevention (general referrals), Community Physio (GP Referrals only), and Rapid Assessment (which can only be referrals from health and social care professionals). Falls Prevention Service referrals should be made by visiting http://www.lancashirecare.nhs.uk/Community-Rehab completing referral form and attaching this to referral. The referral will be declined if the referral form is not completed in full. The service provides a specialist service for adults who have fallen or who are at risk of falling. Patients are visited in their own home and a multifactorial assessment to check for environmental and other contributing falls risks is completed. People may be assessed by an Occupational Therapist and/or a Physiotherapist. We will discuss options for reducing risks of falls and agree a care plan for any on-going treatment or equipment. We also operate an open referral system through the Main Access Point for NHS community referrals 01772 777999. After receiving your referral we may ring the patient to triage the situation and either allocate directly to a therapist or place referral on our waiting list. We endeavour to see people according to need and within 8 weeks where possible.
